Insurance Information for New Patients
Dr. Nate is a preferred provider for Regence BlueCross BlueShield and Deseret Mutual Benefit Administrators (DMBA).
If you have insurance with any other provider, you will be required to pay for sessions with Dr. Nate at the time of service. We accept cash, credit cards and checks. We will submit claims to your insurance provider for reimbursement to you.
Dr. Nate does NOT accept Medicaid or Medicare.
INSURANCE IS YOUR HEADACHE -- AND OURS!
It is YOUR responsibility to know your insurance policy!
We happily file your claim with your insurance company as a courtesy. Submitting claims does NOT guarantee that your insurance provider will pay for your sessions. Please keep in mind that payment remains your responsibility. We do not enter disputes over insurance benefits. We expect payment in full from you if your insurance company delays processing of your claim for over 60 days. You agree to pay any portion of the charges not covered by insurance. If your insurance company sends payments directly to you, mail or drop off the payment to Choices for Change Counseling and we will happily apply it to your account.
There may be a balance due once insurance pays their portion.
